The head of Tennessee's prison system is to step down following the failed execution attempt of Christa Pike, the state's only female death row inmate. Pike, convicted of a 1995 murder, saw her execution not proceed as planned, prompting the departure of the corrections chief and renewed scrutiny of the state's capital punishment procedures.
Why now: A failed execution prompting a senior official's resignation raises fresh questions about Tennessee's death penalty system.
